Waiver of Liability and Release Form
Pure Grappling LLP
1. **Assumption of Risk**
I, {name}, understand and acknowledge that participating in Brazilian Jiu Jitsu involves inherent risks, including but not limited to physical injury or mental distress. I voluntarily choose to participate (or, if signing up on behalf of my child, choose to have them participate) understanding these risks.
2. **Medical Clearance**
I confirm that I know of no medical reason why I (or, if signing up for my child, my child) should not be able to participate. If any medical conditions are known, I have consulted with a doctor who has cleared participation in physical activities such as Brazilian Jiu Jitsu.
3. **Release of Liability**
I hereby release and hold harmless Pure Grappling, its instructors, staff, agents, and representatives from any and all claims, demands, or causes of action, arising out of or in connection with my (or, if signing up for my child, my child's) participation in Brazilian Jiu Jitsu training and activities.
4. **Indemnification**
I agree to indemnify and defend Pure Grappling against any claims brought by third parties arising from my (or, if signing up for my child, my child's) conduct during training and participation in school activities.
5. **Photo and Video Release**
I consent to the use of my image, including photographs and videos, for promotional purposes by Pure Grappling (this does not apply to images of children participating, which are kept private to our club).
6. **Code of Conduct**
I agree to adhere to the school's code of conduct and respect the instructions of the instructors. I understand that failure to do so may result in the termination of my membership without refund.
